Default Request for file rejected

I requested my file from a CRA (not one of the big 3). Included with my request was a photocopy of my drivers license and my SS#. And they repiled with this:

"
Dear Consumer:

In response to your request we have enclosed an original form entitled "CONSUMER DISLOSURE REQUEST" for you to return via mail. I am sending this form because your signature was not present on your former request you recently sent to our office form your consumer report.

At this time we are unable to verify your identity therefore we are unable to proceed further with your disclosure.

Upon return of the completed Request for Disclosure, the consumer Relations Department will be able to provide you with the information you requested within a 24 hour tiem frame. Please be sure to send the original copy back to our office via mail. If you should have any additional questions or concerns, pleaes contact us as .....

And yes they spelled it "dislosure" the first time.

Are they allowed to pick what form they want people to fill out to get thier file, and require a signature? Or is that a violation of the FCRA?

Who did you request it from?

Some CRAs are much more difficult to deal with. They want to make it as difficult as possible for you. It's like a rebate, they don't want you to fill it out.

They are just trying to be "safe" and a PITA at the same time. Probably just one of their policies.
It's ok to fill out the form and sign it. It's not a violation for them to ask for it.
